AUT5000 Registration Form DRIVER EDUCATION REGISTRATION WILL BEGIN JUNE 1, 2010 The course combines the benefits of practical driving along with defensive driving techniques for safe and responsible operation of a motor vehicle. All students must be hold a valid New York State learner’s permit or driver’s license. Successful completion of this course will qualify the student for DMV Certificate of completion (formerly blue card). The fee to replace a lost certificate is $20. Classes will consist of 24 hours of vehicle instruction including 6 hours of behind the wheel driving, and 24 hours of classroom instruction. The program includes a written exam and driving skills test. Students are required to attend all sessions. This requires a commitment on the part of students, parents and teachers in order to fulfill the New York State course requirements. All missed classes must be made up and a fee of $45 will be charged for any missed class or drive time. A student with three absences will be removed from the program. Registration forms for this program are available in the IHS Main Office, the Community Education Office (located in IHS, room 2094), WICSD district office & online. Enrollment is limited to 32 students.
